Registration
To become involved with the Health and Spirituality Program just mention this to one of your health care practitioners at the clinic. As you discuss your overall health this program may be something you and your health care practitioner feel is helpful. Suggestions will be given of which kind of prayer practices would be most beneficial in relation to you and your health.
An appointment will then be made with a staff member of Minnesota Institute of Contemplation and Healing (MICAH). There will be one or more consultations until you feel comfortable beginning to integrate the practices into your daily life. Consultations will initially take place at the Integrative Medicine Center, but may also be arranged to take place at the MICAH rural site.
Begin registration by reading the Covenant agreement. Then, register by email or phone or by mail. Once we receive your registration form and payment we will contact you, send you your health questionnaire, and begin your program.

Call our office or send an email (trey.crxpres@midconetwork.com) with the following information:
- Your name
- Your address
- Your primary phone number
- Your alternate phone number
- Date of birth
- Whether you are Male or Female
- Acknowledge that you have read and accept the Covenant Agreement
The MICAH Office hours (located at the First Presbyterian Church in Crookston) are Monday through Thursday 9:30 a.m. - 1:30 p.m., Central Standard Time. If you call after office hours please leave your name, phone number, and the best time for us to return your call.
You will receive a confirmation email from our office.
